Future health today- integrating proactive collaborative diabetes management systems

Project Details

Diabetes and diabetic kidney disease are significant health issues, with most cases diagnosed and managed in the primary care setting. Future Health Today is a quality improvement technology platform for general practice, developed as a collaboration between Western Health and the University of Melbourne. This program is capable of flagging patients with or at risk of these diseases, and making recommendations for further investigations, diagnoses, and management decisions. It is hoped this technology will improve detection and management of diabetes and its complications in general practice clinics in the western suburbs of Melbourne.
