Western Health Chronic Disease Supported Program
On behalf of the Western Health Nephrology and Endocrinology units, VicREN and The University of Melbourne are delighted to invite practices to participate in a new clinical trial focused on preventing or delaying progression of chronic disease in our community with chronic kidney disease and diabetic care as key priorities.
The project is exploring a model of care that combines electronic clinical decision support with enhanced support from the Western Health team and is available to practices situated in the Western Health catchment.
What do you need to do?
The project will ask your GPs to use an innovative clinical decision support technology called Future Health Today (FHT) in their daily practice. FHT will be provided to your practice FREE of charge for the duration of the project. In addition, Western Health Clinicians will provide clinical education and support, access to a chronic kidney disease nurse practitioner and increased diabetes educator support. Project outcomes will be evaluated using data from your practice, securely extracted using the GRHANITE™ tool.
By participating in this project, you will help to improve:
- The diagnosis and management of chronic kidney disease;
- Diabetes and cardiovascular disease across primary and secondary care;
- Collaboration between general practices in the western suburbs of Melbourne and Western Health; and most importantly
- Health outcomes for patients.
This project is funded by Safer Care Victoria via the 100,000 lives initiative and includes a $1,000 payment for participating practices.
