Leadership and Collaboration
Led by Chief Investigators Professor Lena Sanci and Professor Grant Russell, the Transform Project unites a diverse network of partners. The project capitalises on existing partnerships across three distinct regions of Melbourne: Northwest, Outer East, and Southeast. Key stakeholders from various sectors are involved, including:
- Primary Health Networks: North-Western Melbourne, Eastern Melbourne, and South-Eastern Melbourne
- Major Hospitals: Royal Melbourne Hospital, Eastern Health
- Community Health Organisations: cohealth
- Academic Institutions: University of Melbourne, Monash University
- Research Networks: TROPHI, MonREN
- Health Service Partnerships: South-East Metro Health Service Partnership, Better Health Network
Additionally, the Transform Project collaborates with professional bodies such as the Royal Australian College of General Practitioners (RACGP) and the Australian Primary Health Care Nurses Association (APNA), alongside government entities like the Victorian Department of Health and the Victorian Multicultural Commission.
