20% Human albumin solution fluid bolus administration therapy in patients after cardiac surgery

Abstract
After cardiac surgery, patients frequently receive fluid bolus therapy. The aim of fluid bolus therapy is to improve organ perfusion. Fluid bolus therapy may be given as a crystalloid and/or colloid fluid. Albumin, a natural colloid, may improve blood pressure for less volume administered than crystalloid fluids. Giving lesser volumes of fluid is associated with better outcomes. Accordingly, albumin solutions are widely used after cardiac surgery in some countries. Amongst albumin solutions, 4 or 5% albumin is the most frequently used colloid in intensive care units worldwide. However, a greater concentration of albumin with less chloride (20% albumin) has been reported to confer a physiological advantage compared to both crystalloid and 4 or 5% albumin, and a survival benefit in some critical ill patient cohorts.
This thesis reports a program of work. The major trial, HAS FLAIR-II, was a pragmatic multicentre randomised controlled study comparing 20% albumin fluid bolus therapy with standard care in 466 post cardiac surgical patients. This thesis evaluates whether 20% albumin reduces resuscitation fluid requirements, accelerates shock resolution, and influences the incidence and severity of cardiac surgery-associated acute kidney injury. This data has implications for fluid management in caring for patients post cardiac surgery.
