This is a joint research seminar between the Department of Critical Care and the Department of General Practice and Primary Care - on the topic of Transitions of Care for High Risk Patients.
The purpose of this symposium is to share research ideas and support future academic and clinical collaborations. We anticipate continuing a series on this topic in 2024.
